SCCHA’s Continuum of Care (CoC) grants provide rental assistance in conjunction with supportive services for homeless and chronically homeless individuals and families with long-term disabilities resulting primarily from serious mental illness, alcohol and/or drug abuse, or an HIV positive medical condition. Case management and supportive services are provided by SCCHA’s partner agencies in order to help participants maintain stability in permanent housing. These agencies also identify and refer participants to the program.
The program is contingent on annual grant funding from the U.S. Department of Housing and Urban Development (HUD) and is not operated under SCCHA’s Moving to Work (MTW) program authority.
Who Is Eligible
- Must meet Housing Choice Voucher (HCV) program eligibility requirements.
- Participants must be homeless/chronically homeless AND disabled to be eligible (per HUD’s definitions of homelessness, chronic homelessness, and disability).
- Participants must be referred by one of SCCHA’s partner agencies in order to apply to the program.
